All Selected entities (Non-Federal entities including federally recognized Indian Tribes, state/local govenment, college, university, and other nonprofit organizations) are required to submit a yearly Audit Evaluation in accordance with the regulations of OMB Circular A-133. Audit evaluations for fiscal year 2007 must be submitted to ADE Audit Unit by January 14th, 2008.

Those entities that expended $500,000 or more in federal funds during the previous fiscal year must undergo an OMB A-133 audit or submit all documentation, which supports the previous performance of an independent OMB A-133 audit for the fiscal year in question. Even if no federal funding was received during the fiscal year in question, entities are still required to complete this evaluation and send it to ADE Audit Unit. 1) Memorandum: An Arizona Department of Education notification that explains the mandated requirements of entities to complete and submit the Audit Evaluation.

2) Instructions: A two page instructional sheet that guides entities through each step of filling out the two-page Audit Evaluation.

3) Audit Evaluation Form: Page One requires general information from entities such as name, address, and type of organization (e.g., nonprofit, local government, etc.). If a given entity received no federal funds during the fiscal year in question, that entity should check the appropriate box in item 9, and can then skip downloading and filling out Page Two of the Audit Evaluation, and send only Page One to ADE. If the entity received any federal funds, Page Two must also be filled out and accompany Page One of the Audit Evaluation.

Page Two of the Audit Evaluation requires entities to list (by line item) all sources and amounts of Federal funding expended during the previous fiscal year.

4) All federal funds are linked to a specific Catalog of Federal Domestic Assistance number (CFDA). It is required the CFDA number be included on the Evaluation form. Please click the link below to download a complete list of all federal program funds:
