A General Statement of Assurance guarantees accountability to the
United States and the State of Arizona by the recipients
of Federal and State assistance grants. Consistent with
34 C.F.R. Section 76-85, the SUBGRANTEE assures,
if awarded a grant, subgrant, or contract that the SUBGRANTEE
will accept funds in accordance with applicable Federal
and State statutes, regulations, program plans, and applications,
and administer the programs in compliance with all provisions
of such statutes, regulations, applications, policies
and amendments.
A document required to receive federal and state funding
for projects/grants administered by the Arizona Department
of Education (ADE)
Submitted annually; to be returned by May 31st; reminder email in March
Must be signed by Authorized Representatives in Blue Ink
Common Logon UserID (Username and Password) is required

Below are the links for obtaining all information and forms
required for filling out the General Statement of Assurance.
1) FY 2009 GSA: Fiscal Year 2009
General Statement of Assurance is due May 31, 2008. Please
return a hard copy of FY 2009 General Statement of Assurance
directly to the Grants Management Office. Please use blue
ink for the signature(s) on Page 4 through 6 and return
all 8 pages of the General Statement of Assurance. Click

2) Please use the following letter templates to modify
the current GSA or request capture only access. Be sure
to use your entity letterhead and mail a hard-copy directly
to the Grants Management Office. Only President, Chairman,
or Comparable authorized official, Charter Holder Contract
Signer on the current GSA can request add/ remove access
to the Grants Management.
Add
users to GSA : Use this template to add users to
your current General Statement of Assurance (GSA) during
the year. It requires both signatures in Blue Ink from
the person who is being added and requesting person (President,
Chairman, or Comparable authorized official, Charter Holder
Contract Signer). Use your entity letterhead and mail
a hard-copy directly to the Grants Management Office.
Requesting
Capture only Access: Use this template to request
users with Capture only Access. Users with Capture only
Access will be able to do data-entry for applications,
amendments, and completion reports but cannot access the
cash management reports. They cannot submit any reports
to ADE. Use your entity letterhead and mail a hard-copy
directly to the Grants Management Office.
Remove
users: from GSA: Use this template to remove users
from the current General Statement of Assurance (GSA).

3) Below are the links for obtaining all information
and forms required for establishing a new entity through
Arizona Department of Education to receive a grant , adding
a new site, or closing a site.
How
to establish, update, or close a site or entity :
In order to establish a brand new site or update / close
an existing site, Districts and Charters must submit required
documents to its responsible office. Please click here

AZ
W - 9 form: New Charter Schools will submit GSA (General
Statement Of Assurances) and W-9 form to Arizona State
Board for Charter Schools. Any existing entities with
change in warrant address should submit a new W-9 form
to ADE Grants Management Office. Please note that AZ W-9
form is used to generate your payment from Arizona Department
of Education. Payments are normally sent to its warrant
address indicated in entity's W-9 form. Any change in
W-9 form, entity should submit a new W-9 form to Grants
Management Office.
General
Statement Of Assurance: A brand new entity who wishes
to apply for a grant through Arizona Department of Education
should submit a General Statement of Assurance to its
responsible office. (Arizona State Board for Charter Schools
or ADE Grants Management Office)
* Please note that General Statement of Assurance is annual
form. If you wish to apply for FY 2009 grant application
during Fiscal Year 2008 (07/01/2007 - 06/30/2008), you
must have current year GSA on file.
