Online Application

The process of application to be considered as an Eligible Training Provider for placement on the arizonaheat website is two-pronged. When an interested party submits an application, it goes simultaneously to the selected Local Workforce Investment Board (LWIB) and to the Arizona Department of Education (ADE) Workforce Development. The LWIB must approve the Provider and Program(s) before ADE can approve them.

Once you apply online to be an Eligible Training Provider you will be asked to select a Login ID and Password to access the Program application/submittal page, and to access your online information, view program status and make changes. It is the responsibility of the Training Provider to maintain all information and assure all information is current and up to date.

After receipt of Login ID and Password, the Training Provider must log in and submit Program(s) for consideration. See the upper right-hand corner of “Status Summary Page” [your logged-in page]. If you need assistance, click on Help. On left-hand menu, click “Help for Training Providers,” then click “Adding New Programs.”

As part of the process of state level validation of Training Providers, the WIA State Plan requires evaluation of the documentation noted below. You do not need to wait for LWIB Program approval to send the documents to ADE. In fact, it streamlines the process if ADE can be examining license, insurance, financial and grievance documentation while the LWIB is making its determination. If all Provider documentation is in place with ADE, the program(s) may be approved by ADE as soon as the LWIB approves the program(s).

To complete processing of any Training Provider application, submit the following four (4) items of documentation to the Arizona Department of Education Workforce Development Unit:

  • Copy of Provider’s current license from the appropriate Arizona licensing authority
  • Copy of Provider’s Certificate of Liability Insurance, showing $2 million General Liability Aggregate and naming the Arizona Department of Education as an additional insured (use address as noted below)
  • Copy of Provider’s most recent audit or financial statement
  • Copy of Provider’s student policies and procedures, particularly grievance policies and procedures
