Rather, they should reflect student successes and progress toward your goals, as well as the attainment of your goals. No personal names, please.
Verify that selected grades are inclusive of those offered at your school. Select grades added since the 2004-2005 SRC; de-select grades no longer offered at your school.

Areas of Involvement (Council Duties)
Identify up to six school site council areas of involvement. Some options might include Budget, Student Discipline, Personnel Decisions, Instructional Programs/Strategies, Curriculum Development, School Improvement, Promotion/Retention Issues, School Safety Issues, School/Business/Community Relations, Extracurricular Activities.
Average Daily Instruction TimeAs an example, if your average daily instruction time is 6 hours and 15 minutes, you would select 6 for the Hr box, and 15 for the Min box that follows. Hours and minutes will be combined in the professional copy of the School Report Card to read "Daily Hours and Minutes".
English/Reading/Language Arts; Mathematics; Science (includes the discrete sciences – Biology, Chemistry, Earth Science, Physics); Foreign Language; History; Geography; Civics and Government; Economics, Visual Arts; Music (includes vocal and instrumental only).
An Organized subject matter in which instruction is offered within a given period of time and for which credit toward promotion, graduation or certification is usually given. A course consists of knowledge selected from a subject for instructional purposes in the schools. Each period or class taught must be included in the course count.
Social ServicesIdentify up to four social services that are available at or through your school. Some options might include Day Care, DES Services, GED/Literacy Classes, Prenatal/Parenting Assistance, Crisis Intervention, Clothing/Food Banks, Counseling Services, Citizenship Classes, Recreational Activities, Health Services, Adult Education, Adult ESL Classes.

FTEFull Time Equivalency is a whole number or decimal that connotes the portion of the day a person spends fulfilling job duties. Examples – a full-time Elementary teacher = “1” in the Elementary count; a full-time Math and Science teacher = “.5” in the Math count and “.5” in the Science count; a part-time Spanish teacher who only works ¼ of the day = “.25” in the Foreign Language count.
Under NCLB, core academic subject teachers must:
• Hold at least a bachelor’s degree.
• Have full state certification (intern, provisional or standard ONLY) – except for Special Education, charter school teachers are exempted from state certification requirements.
• Have demonstrated competence in their subject areas. Elementary teachers (K-8 self-contained) demonstrate subject matter competence by passing the Elementary Subject Knowledge AEPA (certification test) or through 100 points on the HOUSSE rubric; Middle School and higher teachers (departmentalized, 7 th grade and up) demonstrate subject matter competency through passing the appropriate AEPA; or an advanced degree; or a major/24 hours; or National Board Certification; or 100 points on the HOUSSE rubric; SPED/ELL core academic teachers of record demonstrate subject matter competency by meeting the requirements for Elementary or Middle & higher teachers mentioned above for the core area taught.

Number of Copies of School Report CardsEnter the number of SRC copies to be printed for your school. Maximum number to be printed is 100% of current enrollment.
Open Enrollment in 2004-2005Open enrollment means the students who attended your school from out of your attendance area for 2004-2005. Your attendance clerk knows the number of students enrolled under open enrollment. Do not enter your total student enrollment for 2004-2005 here. Your open enrollment number is usually significantly smaller than your total enrollment. Exception: Charter Schools' total enrollment and open enrollment numbers will be nearly identical since their possible enrollment area is the entire state.
New Schools: Leave item blank.
Be sure to select the area code in the drop-down box. You will have a space to enter the first three digits of your phone number and another space to enter the last four digits. The program will automatically hyphenate the phone number in the final version of your School Report Card. This will also apply to any phone numbers that you enter in the Contacts section (p. 10).
Prefix/Title (Chief Administrator)
Existing Schools:> From the drop-down box, select the prefix preferred (Mr./Mrs./Miss/Ms./Dr.), then enter the name. The job title you selected for 2004-2005 is shown. If this is still correct, do nothing. If the title needs to be changed, select the correct title (or its closest descriptor) from the drop-down menu.
New Schools: Follow the directions, above, and also select the correct job title of your Chief Administrator from the drop-down menu.
Staffing numbers should reflect figures for 2005-2006. If a person serves dual roles in the same school, count that person as "1" in the position the person spends the majority of time. If a person is shared among schools, count that person as a percent of FTE. "Teachers" are defined as having regular or full- or part-time teaching positions in the classroom. "Other Professional Staff" are those who are ADE certified, but do not have a regular teaching assignment (i.e. counselor, psychologist, school nurse).
Note: If you enter no numbers for staff, we will enter zeros in the categories.
Some examples are ensuring student attendance, providing proper clothing and nourishment, transportation commitments, homework support, involvement in school goals and expectations, supporting school policies including dress codes and behavior codes, communicating regularly with the school and your child.
Responsiblities Your School HasSome examples are high academic standards, setting goals and expectations for students, distribution of handbooks/policies/other materials, regular home/school communication (e.g., release date of student report cards).
School NameEliminate the word "School", if necessary, for character fit (maximum 75). Abbreviate words in your school name, if needed; however, do not use abbreviations (St., Ft., Mt. are exceptions) in the first few words of your school name since those are the words people will use to search for your school data.
School SafetyTotal number of incidents that occurred on the school grounds in 2004-2005 that required the intervention of local, state or federal law enforcement (A.R.S. § 15-746.6).
School YearEnter Start and End dates as follows: mm/dd/yyyy, e.g., 8/16/2004, 5/19/2005.

No personal names, please. You may wish to include the Maximum/minimum transporting distances, a description of your school's general transportation boundaries, transportation responsibilities for open enrollment/special education students, expected student behavior, etc.
UnduplicatedA count in which a given student or staff member is represented or counted once in a single category. All counts contained in specific sections of the report are to be unduplicated.
WaiversEmergency Teaching Certificates only. For the purposes of reporting waiver data to the U.S. Department of Education, teachers with provisional or standard certification in ANY area are NOT to be included in the waiver count, even if they are teaching a subject out of field under an emergency certificate. Teachers enrolled in alternate certification programs and/or those who received their provisional or standard certificate by the end of academic year 2004-2005 should also be excluded from the waiver count.
