Welcome to the School Report Card Data Collection Site. From this site you may enter, edit and submit your School Report Card information.
Your School Report Card (SRC) has been organized into eleven pages (Pages 01-11). If you submitted a School Report Card to the Arizona Department of Education (ADE) for 2004 - 2005, these work pages will contain a copy of that information. You may view or print a copy of last years data by clicking on the Document icon
When you have completed all ten pages, click on the Document icon In submitting your School Report Card data, you are authorizing the ADE to publish the information you have submitted. Remember: It is your responsibility to edit for errors, inaccurate information and misspellings before submission of your report card data. The Department will print all information exactly as submitted. After submitting your data, please take a few extra minutes to complete the Feedback and Comments section. If you have any questions about School Report Card content, please e-mail reportcards@ade.az.gov or contact LaDonna Woodworth at the Research and Evaluation Division at 602.542.5022. Password questions should be referred to the Enterprise Office at enterprise@ade.az.gov. Technology-related (hardware/software) questions should be referred to 866.577.9636 or 602.542.7378. GENERAL COMMENTS The following fields: Current Enrollment, both Open Enrollment questions, Copies for Distribution, Safety, School Council, School Year Start and End dates, are mandatory. You will be unable to finalize these pages or submit your data until these fields are completed. Data may be entered on other pages until you obtain this information. Abbreviations may be used for commonly abbreviated words. If an abbreviation will probably not be understood by your parents/community, it is best to spell out the word(s). Character limitations may sometimes preclude this from being possible. Please capitalize the major words in lists. In a two-word hyphenation, follow this style: Two-word Hyphenation. When you see the Glossary You are encouraged to view the Glossary in its entirety before you begin entering data. To do this, click the Glossary button above the Summary Status menu. |
