Welcome to the School Report Card Data Collection Site. From this site you may enter, edit and submit your School Report Card information.

Your School Report Card (SRC) has been organized into eleven pages (Pages 01-11). If you submitted a School Report Card to the Arizona Department of Education (ADE) for 2004 - 2005, these work pages will contain a copy of that information. You may view or print a copy of last year’s data by clicking on the Document icon (PDF File for review). Please review each page by clicking on the page number on the Summary Status menu; then make any necessary changes in that page. Please remember to SAVE any changes you make by clicking the "Save Data and Continue" button for each page. If your school is new this year, you will need to enter data in all fields. Again, be sure to click “Save Data and Continue” on each page.

When you have completed all ten pages, click on the Document icon for the PDF File for review on the Summary Status Menu on the left side of the screen. Here, you may view as a single page all data from pp. 01-11. If you see changes that need to be made, you must go back and make them on the work pages. If you do make changes, again be sure to click “Save Data and Continue” on each page to which you make changes. The final submission button will appear after all sections are complete. You must see “Complete” with a date showing for each page before the Submit button will appear. When all items have been completed, click the “Submit” button. A submission number and message will appear giving you the date and time of submission. At this time, you will be given the option to complete the “Feedback and Comments” section.

In submitting your School Report Card data, you are authorizing the ADE to publish the information you have submitted. Remember: It is your responsibility to edit for errors, inaccurate information and misspellings before submission of your report card data. The Department will print all information exactly as submitted. After submitting your data, please take a few extra minutes to complete the “Feedback and Comments” section.

GENERAL COMMENTS

  • The following fields: Current Enrollment, both Open Enrollment questions, Copies for Distribution, Safety, School Council, School Year Start and End dates, are mandatory. You will be unable to finalize these pages or submit your data until these fields are completed. Data may be entered on other pages until you obtain this information.

  • Abbreviations may be used for commonly abbreviated words. If an abbreviation will probably not be understood by your parents/community, it is best to spell out the word(s). Character limitations may sometimes preclude this from being possible.

  • Please capitalize the major words in lists. In a two-word hyphenation, follow this style: Two-word Hyphenation.

  • When you see the “Glossary” icon next to a boldfaced word(s), it indicates access to a glossary item that has been created with further instructions and information to help you more accurately and completely enter your data. Each glossary item can be viewed by left-clicking on the boldfaced word or on the icon beside it.

  • You are encouraged to view the Glossary in its entirety before you begin entering data. To do this, click the “Glossary” button above the Summary Status menu.
