Claims Selection

Program Year

Select the program year for which you want to view claims.

Month

Select the individual month for which you want to view claims.

The default value is [All], which displays claims for all months of the selected program year.

Claim Status

Select the claim status of the claims you want to view.

The default value is [All], which displays all claims for the selected program year and month.

Search For

Specify whether you are searching for a site or a sponsor. The default choice is Sponsor.

Note

ADE can use the search to find either sponsors or sites, however, sponsors cannot view claims for other sponsors. If a sponsor searches for another sponsor, the message "No Results to Display" displays.

Search by Name

To search for a claim by the site or sponsor name:

1. Select the Program Year, Month, and Claim Status you want.

2. In the Search For list, select whether to search for a Sponsor or Site.

3. Select Search by Name.

4. Select Begins With, Ends With, or Contains to specify the position of the letters within the name.

5. Type all or part of the legal name of the sponsor or site you are searching for.

6. Click Go.

Search by CTDS

To search for a claim by the site CTDS number or sponsor CTD number:

1. Select the Program Year, Month, and Claim Status you want.

2. In the Search For list, select whether to search for a Sponsor or Site.

3. Select Search by CTDS.

4. Type the unique County/Type/District/Site identifier used for this sponsor or site throughout Arizona Department of Education's business applications.
   
   Enter the number with no separating punctuation or spaces.

ABCDEFGHIJKLMNOPQRSTUVWXYZ 0-9

To use the index of names, click the first letter of the sponsor or site name.

Notes

• The index only displays to Arizona Department of Education users.

• Select 0-9 if the name begins with a number.
