General Statement of Assurance

PLEASE NOTE: The deadline for submitting the FY 2012 General Statement of Assurance is June 10, 2011.
The FY 2012 General Statement of Assurance was amended on April 25, 2011 to include a Section E.

A General Statement of Assurance guarantees accountability to the United States and the State of Arizona by the recipients of Federal and State assistance grants. Consistent with 34 C.F.R. Section 76-85, the SUBGRANTEE assures, if awarded a grant, subgrant, or contract that the SUBGRANTEE will accept funds in accordance with applicable Federal and State statutes, regulations, program plans, and applications, and administer the programs in compliance with all provisions of such statutes, regulations, applications, policies and amendments.

1) FY 2012 General Statement Of Assurance

  2.   Fiscal Year 2012 GSA Due Date - June 10, 2011
  3.   Mail a hard copy document directly to the ADE Grants Management Office
  4.   All original signatures must be affixed in blue ink.
  5.   Section A, Section C, Section D, and Section E must be signed by :
    •   Districts: Board President, Superintendent
    •   Charters: Charter Contract Signer
    •   Other Entities: Comparable authorized Official

  7.   Click here to only download the FY 2012 Section E that was amended on 4/25/2011. (Only use if your entity already submitted a hard copy to ADE without the amended Section E)

2) A brand new entity who wishes to apply for a grant through the Arizona Department of Education for the first time should :

  1.   Apply for a CTDS number online. CTDS stands for County, Type, District, and School.
    To apply for a CTDS number, go to the ADE website at http://www.ade.az.gov/schoolfinance/FAQs/CTDS_District/ and follow the directions on this page.
    If you are a new site, please use CTDS# 00-00-00-001 when completing the form and select "School Finance" as your LEA Account Analyst
  2.   Submit the FY 2012 General Statement of Assurance to the ADE Grants Management Office. If you wish to have access to the secure Grants online prior to July 1, 2011 (SY 2012), please also submit the FY 2011 General statement of Assurance.
  3.   Submit a W-9 to the ADE Grants Management Office. Please note that AZ W-9 form is used to generate your payment from Arizona Department of Education.

3) Change Form

  1.   Click here to download Change Form.
  2.   ADD / Remove Grant Users after submitting annual General Statement of Assurance
  3.   Mail a hard copy document directly to the ADE Grants Management Office
  4.   All original signatures (Requesting Person and New GSA Signer) must be affixed in blue ink.
  5.   Requesting person must be :
    •   Districts: Superintendent or Business Manager
    •   Charters: Charter Contract Signer
    •   Other Entities: Comparable authorized Official

4) Grants User Access

  1.   GSA Access : Users can create, edit, and submit applications, amendments, cash management reports, and completion reports.

  2.   Capture Only Access : Users can create and edit applications, amendments, and completion reports.

  3.   Remove User Access : LEA must select one of the following options to remove a user or provide a specific direction.
    •   Cancel grants management access only - A user will still have an active common logon account but will not be able to access the secured grants management site.
    •   No longer employed - A user's common logon account will be completely deleted.
    •   Retired - A user's common logon account will be completely deleted.


    • * Please be informed that the Grants Management Office cannot add or remove any other common logon applications other than Grants Management Application indicated in your submitted GSA or Change Form.
      * Our office cannot delete a user's common logon account completely if a requested user has an access to CNP web. Adding or removing CNP web access must go through the ADE Health and Nutrition.
  4. Annual General Statement of Assurance only gives users to access the secured grants management site. For any other common logon application requests, please contact the ADE Support Center at enterprise@ade.az.gov or (602) 542 - 7378.

5) Updating an existing entity or Payment Info with ADE

  1.   To update or close an existing site or entity, go to the ADE website at http://www.ade.az.gov/schoolfinance/FAQs/CTDS_District/ and follow the directions on this page.


  2.   Any existing entities (Not applicable to Districts) with change in Tax ID, Legal Name, Entity Type, Minority Business Indicator, Main Address, Remittance Address, or Contact Information should submit a new W-9 form to ADE Grants Management Office.
